Position Summary:

This position will ensure all aspects of a client’s residential needs are met.  Will assist with acquiring relevant skills, promoting an improved quality of life, enhance housing stability and support long-term housing retention through intensive case management supports.

Housing Assistance:

Ensure all intake documentation is completed, recorded and filed.

Assist participants in maintaining good tenancies

Respond to resident issues and concerns as required

Record statistical data and financial data as required.

Programming and Services:

Develop appropriate programming and supports for clients.

Facilitate programming with other staff

Respond to needs of clients and co-ordinate required services.

Provide clients with social service connections needed to obtain identification, OW, employment, counseling, education treatment etc.

Assist in court programs and adult services supports as required (Direct Accountability and Bail program).

Assist in other duties as assigned; this may include administrative tasks and light cleaning in the office.

Services to be Provided:

Support clients who have been accepted in the Home for Good Program

Engage with clients and develop a service plan

Initiate a support service plan that will address safety, basic needs, relationships, daily activities and life skills, social and community integration, risks and crisis triggers, managing tenancy, economic and wellness opportunities, and goals for greater independence

Conduct regular home visit in order to support clients to maintain their housing unit in good repair by monitoring levels of cleanliness, teaching or reinforcing life skills, and referring to community supports

Assist clients with required activities of daily living.

Education and Experience:

Post secondary education in humanities and/or social services

Work related experience in corrections, residential housing programs, homelessness, mental health, poverty, community development or corrections

Details:

Full time one-year contract starting January 2019.

Hours are 8:30 to 4:30 Monday to Friday. Possible evenings and weekends.
